This webinar provides the latest information and tools regarding health care planning and guardianship. We will introduce a NEW Person-Directed Planning Checklist to assess and protect an adult’s decision-making skills and build in needed supports, accommodations and alternatives to avoid Guardianship and Conservatorship. ·        New Adult and Supportive Person Tool Kit for adults with cognitive impairments and complex health needs.

·         The Guardianship Process: When and how to initiate, modify or terminate a Guardianship with new on-line resources.
