This webinar provides the latest information and tools regarding health care planning and guardianship. We will introduce a NEW Person-Directed Planning Checklist to assess and protect an adult’s decision-making skills and build in needed supports, accommodations and alternatives to avoid Guardianship and Conservatorship. 

Advance care planning, decision-making capacity, guardianship, and patient-directed care are increasingly important considerations for clinicians providing care in the home setting. This webinar will provide an overview of the health care planning process, practical tools to support patient autonomy, and strategies to help individuals maintain decision-making authority through the use of supports, accommodations, and alternatives to guardianship whenever possible.

Participants will be introduced to new resources, including the Person-Directed Planning Checklist and Adult and Supportive Person Toolkit, designed to assist clinicians and interdisciplinary care teams in supporting adults with cognitive impairments, serious illness, and complex health care needs. The session will also review the guardianship process, including when guardianship may be appropriate and available options for modification or termination. In addition, attendees will receive an update on the Massachusetts transition from MOLST to POLST, scheduled for implementation in April 2027.

This content is particularly relevant to clinicians practicing in home health and community-based settings, where understanding patient goals, decision-making capacity, advance care planning, and legal considerations is essential to delivering person-centered, coordinated care. Participants will leave with practical tools, resources, and knowledge that can be incorporated into clinical practice to support patient choice, care planning, and interdisciplinary collaboration.
