Join us for a unique learning opportunity as we explore issues and lessons learned in delivering community-based palliative care services.
Hear from national expert Constance Dahlin, MSN, ANP- BC, ACHPN, FPCN, FAAN. Co-Director of the Palliative APRN Externship. Ms. Dahlin is Co-Director of the Palliative APRN Externship; PhD in Palliative Care, Master of Science, and Graduate Certificate; consultant to the Center to Advance Palliative Care; and a Palliative Nurse Practitioner for Salem Hospital.
Constance will lend her knowledge of Palliative Care services on a National and State level. Providing guidelines and practical tools and resources. A Panel of Agencies will share their experiences and programs.
