The Hospice Outcomes and Patient Evaluation (HOPE) tool represents a major step forward in how hospices assess and document patient care. Rather than relying on retrospective chart reviews, HOPE captures information in real time—tracking each patient’s needs, symptoms, and preferences from admission through discharge.
By collecting consistent, detailed data, HOPE enables providers to improve quality of care, demonstrate compliance, and strengthen overall performance. To succeed, agencies will need to adjust internal workflows, train clinical staff, and ensure accurate electronic record submission.
