Public Reporting Update: October 2025 Home Health Quality Reporting Program Refresh Now Live

CMS resumes public reporting following federal shutdown, releasing updated HH QRP performance data across Medicare.gov and the Provider Data Catalog.

Following the federal government shutdown, the Centers for Medicare & Medicaid Services (CMS) has resumed routine data updates. As of November 20, 2025, the October 2025 refresh of the Home Health Quality Reporting Program (HH QRP) is now live on both the Medicare.gov Care Compare tool and the Provider Data Catalog (PDC).

The update includes a broad range of reporting periods across key quality measures, including OASIS-based clinical outcomes for Q1–Q4 2024, claims-based measures such as Discharge to Community and Medicare Spending Per Beneficiary through Q4 2024, 30-day potentially preventable readmissions through Q4 2024, and within-stay potentially preventable hospitalizations for Q1–Q4 2024. Updated HHCAHPS survey data are also included for Q2 2024 through Q1 2025.

CMS has temporarily suppressed two measures in this refresh—Transfer of Health Information to the Provider and Discharge Function Score—due to technical issues. Both are expected to return with the January 2026 update after full data validation.

This refresh includes:

  • OASIS-based quality data: Quarter 1–Quarter 4, 2024
  • Claims-based measures:
    • Discharge to Community and Medicare Spending Per Beneficiary: Q1 2023–Q4 2024
    • Potentially Preventable 30-Day Readmissions: Q1 2022–Q4 2024
    • Within-Stay Potentially Preventable Hospitalization: Q1 2024–Q4 2024
  • HHCAHPS survey data: Quarter 2, 2024–Quarter 1, 2025

Temporary Measure Suppressions: Due to technical issues, CMS has suppressed results for two measures in this cycle:

  • Transfer of Health Information to the Provider
  • Discharge Function Score

Both measures are expected to return in the January 2026 refresh after full data validation. Home health agencies can review their updated public quality profiles on Medicare.gov Care Compare and the  Provider Data Catalog .
