CMS Flags Advance Care Planning as Potential Future Home Health Quality Measure

Advance Care Planning measure under review for Home Health QRP; no hospice measures included

CMS has released its 2025 Measures Under Consideration (MUC) list, which outlines quality measures the agency is evaluating for possible future use across Medicare programs.
For home health, only one measure is under consideration: Advance Care Planning (ACP) under the Home Health Quality Reporting Program (QRP). There are no hospice measures included on this year’s MUC list.
If adopted in the future, the proposed ACP measure would assess whether patients age 18 and older have either an advance care planning document or documentation of an advance care planning discussion that resulted in a decision, recorded in the electronic health record by the time of discharge.
Inclusion on the MUC list does not mean the measure will be implemented. However, measures that advance through this process often appear later in CMS’s proposed payment rules and may ultimately become publicly reported quality measures.
CMS is accepting public comments through Tuesday, January 6, providing an opportunity for home health providers to weigh in on measure feasibility, documentation burden, and operational considerations.
