What agencies need to know before the June 15 deadline

CHIA has released updated guidance for the FY2025 Home Health Cost Report, including new submission requirements and stricter documentation standards. Here’s what providers need to do now to stay compliant.

The Center for Health Information and Analysis (CHIA) has released its FY2025 Home Health Cost Report guidance, with submissions due by June 15 under updated requirements outlined in 957 CMR 6.00.

What’s Changed

Agencies must now complete the report using a standardized Excel template with built-in financial reconciliation. Submissions must be emailed and include:

  • Financial statements
  • Medicare cost report (CMS-1728)

CHIA has emphasized that strict naming conventions and complete documentation are required. Incomplete or improperly formatted submissions may delay acceptance.

Exemptions Still Apply

Limited exemptions are available for agencies with:

  • Minimal operations
  • Ownership changes
  • No MassHealth participation
  • Less than $50,000 in MassHealth revenue

However, agencies must still submit an exemption form to qualify.
