Community-based palliative care is quickly becoming a core part of the care-at-home continuum and the focus of today’s workshop in Medway.

Thank you to our member, Salmon Health, for hosting providers from across the state for this conversation, led by national expert Constance Dahlin. The session focused on how to design and sustain programs that meet patients where they are, at home and across care settings.

A central theme: palliative care must be clearly defined by each organization, not confused with hospice, and delivered based on need rather than prognosis.
