What Providers Are Facing Now—and What Comes Next

Feedback from recent Regional Member Meetings highlights mounting pressure across reimbursement, workforce, and regulatory environments.

Nearly 100 members joined across two locations, sharing consistent challenges that point to broader system-level strain—and the need for coordinated response.

Nearly 100 members participated in last week’s Regional Member Meetings, and the message was consistent: providers are operating under growing pressure across multiple fronts.

Agencies reported ongoing challenges with billing and authorizations, continued hospital discharge delays, and increasing strain tied to Electronic Visit Verification (EVV) as requirements tighten and claims become more closely tied to visit data.

Workforce instability remains a critical concern. Immigration-related uncertainty is contributing to absenteeism and turnover in an already fragile labor market, compounding staffing challenges across the care continuum.

Flat MassHealth rates, combined with rising operating costs, are creating sustained financial pressure—raising concerns about long-term access and system stability.

System pressure is building across multiple fronts

Providers emphasized that these issues are not isolated. Reimbursement constraints, workforce disruption, regulatory requirements, and hospital throughput challenges are all converging at the same time.

Licensure remains a priority, but progress is unfolding within a broader environment of state and federal fiscal constraints, making timing and implementation increasingly complex.

What this means for providers

The takeaway is clear: these are system-level challenges, not individual agency issues. Provider input will continue to shape how the Alliance responds through advocacy, education, and operational support.

Ongoing engagement—including participation in meetings, working groups, and feedback channels—remains critical as policy and operational decisions continue to evolve.
