This session explores what happens in the moments between visits, when patients and caregivers are without immediate clinical support, questions arise, confidence falters, and adverse events often occur. Participants will examine how thoughtfully designed AI-enabled triage, outreach, and escalation workflows can extend continuity beyond traditional encounters by reinforcing guidance, identifying concerns earlier, supporting caregiver decision-making, and helping teams respond more effectively between touchpoints. Through real-world examples, the session will illustrate how AI can enhance safety, promote stability, reduce preventable utilization, and strengthen the patient and caregiver experience during the gaps where care is often least visible, but most impactful.
