This webinar provides the latest information and tools regarding health care planning and guardianship. We will introduce a NEW Person-Directed Planning Checklist to assess and protect an adult’s decision-making skills and build in needed supports, accommodations and alternatives to avoid Guardianship and Conservatorship. Participants receive new tools and on-line free resources. The webinar includes-

 

·         Who’s Your Agent? health care planning process to engage adults in simple to serious illness conversations with new tools.

·        New Adult and Supportive Person Tool Kit for adults with cognitive impairments and complex health needs.

·         New Person-Directed Planning Checklist to make a plan and build in supports and accommodations to avoid Guardianship and Conservatorship.

·         The Guardianship Process: When and how to initiate, modify or terminate a Guardianship with new on-line resources.

·         Update on MOLST to POLST Transition going live April 2027.

·         Q/A- share comments and questions to address challenges.
