This engaging course aims to improve the clinical judgment, regulatory fluency, and interdisciplinary coordination skills of Certified Home Health Clinicians. Using a progressive patient case study, participants will apply core principles of case management to real-world scenarios, gaining practical understanding of care planning, documentation, and communication strategies that promote quality and compliance.

Whether you're new to the role, refining your skills, or mentoring others, this course provides practical strategies to align care delivery with CMS Conditions of Participation, HHVBP goals, and the principles of patient-centered care.

The course supports professional growth, survey readiness, and improved patient outcomes.

Learning Objectives

Upon successful completion of this course, participants will be able to:

Interpret CMS Conditions of Participation as they apply to Case Management
Apply best practices in assessment, coordination, and documentation
Strengthen interdisciplinary communication across the care team
Analyze patient scenarios to identify effective case management strategies
Describe the role of case management in achieving quality outcomes and HHVBP success
Integrate a patient-centered care approach into daily practice

Speaker:

Kim Portello, PT, COS-C Kim Portello is a Senior Healthcare Leader and Certified OASIS Specialist with more than 35 years of experience in home health and hospice care. Kim is currently the Associate Director of Regulatory and Clinical Affairs at the Home Care Alliance of Massachusetts. Before her current role at HCA, Kim held progressive leadership roles at Hebrew SeniorLife, including Rehab Manager, Director of Clinical Operations, and, most recently, Director of Quality and Patient Safety. Kim is recognized for advancing compliance, quality, and patient outcomes, including leading deficiency-free Department of Public Health surveys, improving CMS star ratings, and achieving success in Medicare audits. She has extensive expertise in regulatory compliance, risk assessment, and staff education and has partnered with clinicians and leadership teams to drive organizational performance. A licensed physical therapist, Kim is passionate about empowering staff and fostering a culture of continuous improvement in post-acute care.
